There should be individual accountability of staff members when such incidents of breach of duty of care occurs. They should face jail sentences as a deterrent. Loss of pensions. It's because there's NO accountability!!
The reality is, the prison system in America are some of the worst in the world. Look at places like Sweden and make the comparisons. Their prison cells look like tiny apartments in most of their prisons and they focus on mental health and rehabilitation. The system we has ensures there is a constant supply of men coming and going. Prisons make money for the state. Don't let them fool you.
